If you have generally healthy eyes and a VSP or NBN vision plan, choose “Vision Plan Appointment.” It includes a refraction test (which isn’t covered by insurance) and screenings for eye diseases that may require a separate medical visit.
If you have a MEDICAL eye concern and have not been seen by any of our providers in the last 3 years, choose “New Patient.”
If you have a MEDICAL eye concern and have been seen by any of our providers in the last 3 years, choose “Existing Patient.”
If you are ready to move forward with LASIK and find freedom from glasses and contacts, schedule here. *If it is found you are not a candidate for LASIK, the visit will be billed as an eye exam. If you do move forward, the visit will be part of your surgical fee.
If you are interested in moving forward with Refractive Lens Exchange (RLE), schedule here. *If it is found you are not a candidate for RLE, the visit will be billed as an eye exam. If you do move forward, the visit will be part of your surgical fee.
If you have been diagnosed with cataracts and would like to have surgery, schedule here. *If it is found you are not a candidate for cataract surgery, the visit will be billed as an eye exam. If you do move forward, the visit will be part of your surgical fee.
